Dear ZUSTers,

We invite you to participate in a short self online survey regarding student mental health by using the CCAPS-Screen, which is a mental health screening instrument for the general student body that assesses the most common psychological problems experienced by college students. It aims to better understand your mental health condition and keep a healthy self-care. At the same time, it provides valuable suggestions for us to offer more appropriate support, and service to you.

l Take 3 minutes to complete 37 questions.

l Brief screen for depression, generalized anxiety, social anxiety, frustration/anger, eating concerns, alcohol use concerns, family distress, and academic distress.

l Receive your results immediately after you submit the questionnaire.

l Please remember that a screening is not the same as a diagnosis.  If you are concerned about your results, please call the Counselor (0571-85070065) to schedule an appointment. Feel free to make an appointment!

1. If you use a computer, please click the following link.

2. If you use a mobile device, please scan the QR code to join the screen.

3. Complete the questionnaire according to the guidance.

4.Please click

(submit)  when you finish all the questions until the results page displayed.


1. Do not fill out the questionnaire too quickly or randomly, otherwise it may be judged as invalid or abnormal by the system, and it is required to do it again.

2. Do not refresh the web page to prevent data loss.
